Medical requirements Under the Operator Licensing and Vehicle Control Regulation, you must disclose any medical or physical condition that could affect your ability to safely operate a motor vehicle. Examples include: diabetes, seizure disorders, heart or respiratory conditions, cognitive or neurological impairments, musculoskeletal or psychiatric conditions, sleep disorders, or loss of mobility/limbs. If you have such a condition, you must submit a Driver Medical Intake Form.
• Forms are available from an Alberta registry agent office, your doctor, or a nurse practitioner.
• The form must be completed by a doctor or nurse practitioner.
